Compressive-distractive device for the mandible.

 

Description

Offered device and methods can be used in different areas of medicine: face plastic and reconstruction after trauma and gunshot, due to malformation and congenital deformitice, in oncology etc. At first compressive-distractive device (CDD) has been constructed during Afghanistan war for wounded treatment with combined gunshot bony and soft tissue defects of the lower third of the face. There are 5 methods of the mandible osteoplasty were elaborated. After debridement, the mandible stumps are approximated, fixed with CDD, wound becomes less and may be sutured. In four methods of the 5, closed osteotomy one or both the mandible stumps is done for small mobile fragments formation. After short time compression in the osteotomy place, immature callus is distracted in this region. During distraction, bone callus is formed and the defect filled between the fragment and stump. The soft tissue are repaired at the same time. Now the CDD and methods are used for patients treatment with the mandible deformitice and defect any origin (congenital deformitice, malformation, oncology, any resection etc.).

Benefits

Advantages of using the compressive-distractive device and methods:

  • the compressive-distractive device fixes the mandible stamps and fragment very strong which is very important for regeneration and mastication;
  • it can restore straight and curve bone as the mandible;
  • CDD eradicate defect of the mandible up to 15 cm lengthening and more;
  • the method is bloodless;
  • minimally traumatic procedure;
  • the device permit to avoid multiple extensive operation procedures on both soft and bony tissues;
  • CDD secures substitution of the combined soft and hard tissue defect in one stage;
  • skin defect is replaced with face skin and therefore it is very aesthetic;
  • nerves, muscles, mucous membrane, salivary duct and vessels grow in the distraction process with CDD;
  • the method is alternative of routine methods;
  • as a routine method it does not require a bone graft or an implants with their unpredictable outcome (assimilation, resorption, rejection);
  • CDD decreases the duration of treatment three- to fourffold.

Project development

A several years ago 50 CDD were produced, and use for patients treatment up to now. New device was designed but does not made. However different packs (units) were prepared from titanium and tested with excellent result.
